We’re here for you and can schedule an appointment 24 hours a day, 7 days a week. If the office is not open, we can refer you to one of our on call Doctors at your nearby hospital.

Please call our office in advance to schedule an appointment. If you are unable to keep your appointment, kindly call our office as far in advance as possible

Insurance & Billing

To ensure proper authorization and payment by your insurance, please bring the following:

  • Present and current insurance card
  • Photo ID
  • Referral from physician (if any)
  • All necessary authorizations

Our office accepts most insurance plans including HMO, PPO and POS plans. The following is a list of the more common plans we accept: Aetna, Anthem Blue Cross, Cigna, Healthnet, Medicare, Oxford Health Plans, and United Health Care. We also accept personal injury, auto accidents, and workers’ compensation.

Payment for all office services is required at the time such services are rendered. We will bill your insurance company; however payment for deductibles, co-insurance, and co-payments are your responsibility. We will help in every way to assist you in the insurance claim process.

You are responsible for co-pays at time of service.

Additional charges that may not be covered include the purchase of equipment, supplies, and vitamin supplements. Medicare and most private insurance companies do not pay for exercise equipment.

Cash and checks are accepted.
